How to find a patient's GP practice in MediStack, and what to do when it isn't listed or the address is wrong.

Finding a practice

In the patient's demographics, use the GP Clinic box and start typing the practice name. Practices are listed under their official NHS name, and the search matches on that name or the practice's ODS code.

Patients often give the local or former name of their surgery, which will not match. If a search returns nothing, look the surgery up by address in the NHS ODS directory at odsdatasearchandexport.nhs.uk to get its official name, then search MediStack again for that name. For example, a practice a patient calls "Southcoates Medical Centre" is listed as "City Health Practice Ltd" at the same address in Hull.

Searching by address or postcode alone will not return results — use the official name or the ODS code.

Once the practice is selected, its GPs are listed underneath so you can pick the individual doctor.

The practice isn't found, or the address is wrong

If you can't find a practice, check the NHS ODS directory at odsdatasearchandexport.nhs.uk and search by name or address to confirm the correct details.

Adding a GP or surgery

Use the ODS details (including the practice's ODS code) to add the GP or surgery. If you're unable to add it, contact [email protected] with the practice name, address and ODS code.
